Heart Failure Project Map – East of England

The East of England Heart Failure project map provides a visual overview of current and planned heart failure projects across the East of England. It aims to support networking and collaboration amongst teams working on similar projects and/or connecting to projects of interest. The map also has the potential to support the identification of gaps and opportunities for scaling up successful initiatives, ultimately contributing to the goal of improving cardiovascular health across the region.
